Skilled Nursing

Infusion Therapy

Our infusion therapy nurses are skilled, registered nurses who understand the special requirements of infusion therapy.

Speech Therapy

Our medical social workers help patients and their families/caregivers with the adjustment that often follows the impact of illness. These services are performed in the patient’s home under physician’s orders.

Communicating with home health professional staff regarding patient’s psychosocial assessment and collaborating with necessary sources to meet the patient’s needs

The HOPE Program
HOPE, Homecare Options for Palliative Enhancement, is our palliative specialty program that is designed to serve patients needing symptom control and supportive care of an end stage illness in the home setting. Our highly trained, multidisciplinary team works together to help patients achieve their optimum potential level of comfort and functional ability, while remaining in their home.

Mother/Child Program
The Healthy Families Program is a mother/child program provided by SCCH Home care. Our registered nurses are highly qualified and trained professionals who offer obstetrical follow-up for mothers and their new babies as well as prenatal visits when necessary. They understand the importance of a smooth transition from pregnancy to motherhood.

Homecare provides one home visit after mother and baby are discharged to insure comfort and clinical support parents and infants after their stay at Smyth County Community Hospital.

This program is provided as a service to all new parents to make the transition from hospital to home a pleasant experience for the entire family.
